You do not need to fill out my booking form if you have active membership(s) with the following;
P411 (Must have 2 or more OK's),
TER, Eccie, RS, Ave-X, TNA or HX please contact me there.
If you DO NOT have a membership with before-mentioned websites, please be prepared to provide me with the names and websites of providers whom you have seen before.
If you CAN NOT provide a reviewed provider as a reference Please submit your work info.
For Example ; Linkd'In , Buisness website, Buisness card, Check Stub, Work Personal Email. Etc.
